Procedures for notification of purpose of use and disclosure of personal information
SOFTBANK TELECOM sends its ‘Request for disclosing Personal Information’ and the Guidebook with address for sending Request Form and fee amount to an applicant who requests the disclosure of information at the relevant contact.
The applicant who received such Request Form pays the fee specified in the Guidebook and returns completely filled Request Form to the address together with ID verification document designated by SOFTBANK TELECOM. If the request is made by an agent, such agent is also required to send ID verification document of their own.

SOFTBANK TELECOM starts procedures upon receiving the completed Request Form with ID verification document and confirming the fee paid at the contact designated by SOFTBANK TELECOM.
Procedures for corrections and suspensions of use of Personal Information
SOFTBANK TELECOM sends its ‘Request for corrections of content of Personal Information’ with address for sending Request Form, and the Guidebook which includes a sample of document to describe a reason for requesting corrections to an applicant who requests the corrections of information at the relevant contact.
The applicant who received such Request Form returns completely filled Request Form to the address together with ID verification document designated by SOFTBANK TELECOM and a document which describes a reason for requesting corrections. If the request is made by an agent, such agent is also required to send ID verification document of their own.
SOFTBANK TELECOM starts procedures upon receiving the completed Request Form with ID verification document and the document which describes a reason for requesting corrections at the contact designated by SOFTBANK TELECOM.
Fee for disclosure and notification of purpose of use of Personal Information and payment method
The fee for disclosure and notification of purpose of use of Personal Information is ¥ 1,600 for each request. The fee shall be paid in money order (fixed amount postal money order).
- SOFTBANK TELECOM may charge such fees in case when the purpose of use is not notified. SOFTBANK TELECOM also may charge fees for disclosure if requested personal information is not held by SOFTBANK TELECOM or cannot be disclosed due to any reason after SOFTBANK TELECOM's survey.
